Hospice Nurse Specialist

The Hospice On-Call Nurse is a critical member of our interdisciplinary team, responsible for providing high-quality care to patients and their families outside of regular business hours. This role requires a skilled and compassionate nurse who can assess patient needs, provide guidance, and deliver care in a timely and effective manner.
Key Responsibilities:- Respond to calls from patients, families, caregivers, and referral sources in a timely and professional manner.
- Assess patient needs and develop a plan of care in collaboration with the interdisciplinary team.
- Provide guidance and support to patients and their families, utilizing critical thinking and nursing expertise.
- Document all calls, interventions, and care plan changes in accordance with company policy and procedure.
- Collaborate with the hospice medical director and attending physician to implement changes to the plan of care as needed.
- Provide bereavement resources to families as appropriate and respond to referrals calls, providing information regarding hospice care and screening calls for appropriateness.
- Perform patient admission visits as assigned by supervisor and report concerns to the hospice supervisor on-call.
- Participate in the agency's on-call rotation as prescribed by the needs of the agency to provide nursing service to clients when required outside of regular office hours.
- Graduate of an accredited school of nursing.
- Valid license as a Registered Nurse and current Basic Life Support certification.
- Valid state Driver's License and reliable transportation.
- Minimum one year of recent professional nursing experience, preferably in hospice or home health.
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
- Computer skills, including Microsoft Word and email, required.
The On-Call Nurse will work in a variety of settings, including patients' homes, skilled nursing facilities, and residential care facilities. The role requires the ability to travel to customer locations, some overnight stays, and exposure to unpleasant weather. The nurse may be exposed to viruses, diseases, and infections from patients and specimens in the working environment.
The On-Call Nurse will be required to work under some stressful conditions to meet deadlines and agency needs, requiring excellent problem-solving skills and the ability to cope with emotional stress.
