Job Type and DescriptionWe are seeking a motivated and passionate Per Diem Physical Therapist to treat patients in Home Care. As a Physical Therapist, you will work with patients to restore overall function and independence in the home setting.
Responsibilities- Plan, prepare, and carry out individually designed programs to provide skilled therapy to patients.
- Develop a plan of care/goals in consultation with the physician, family, and patient.
- Timely and accurate documentation of visits.
- Effectively communicate discharge with family members and other team members.
- Physical Therapist Program Graduate.
- NY Physical Therapist License or eligible.
- Evaluates a patient's need for physical therapy and develops an appropriate treatment plan.
- Provide appropriate care and treatment to patients based on physician care plan, agency policy, and professional standards of care.
