We are seeking a skilled Registered Nurse to join our Care Management team at River City Center in Decatur, AL. As a Care Management RN, you will play a vital role in providing high-quality patient care and collaborating with our interdisciplinary team to develop effective care plans.
Key Responsibilities- Assess and Develop Care Plans: Perform comprehensive nursing assessments and work with the care team to create individualized care plans that address the unique needs of our patients.
- Collaborate with the Care Team: Communicate effectively with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Delegate and Supervise: Delegate tasks and responsibilities to support staff and supervise their implementation to ensure high-quality patient care.
- Monitor and Evaluate: Continuously monitor patient progress and evaluate the effectiveness of care plans, making adjustments as needed to minimize rehospitalizations and improve patient satisfaction.
- Education and Licensure: Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- Certifications: CPR Certification is required.
- Competitive Compensation: Pay Target: $36 - $40 per hour.
