Clinical Navigator

PAM Health, LLC is seeking a highly motivated and results-driven Clinical Navigator to join our team. As a Clinical Navigator, you will play a critical role in cultivating referral relationships, managing assigned territories, and completing patient assessments. Your sales and analytical skills will be essential in making sales contacts and analyzing referral and admission data from assigned territories.
Responsibilities- Work collaboratively with the business development team to facilitate referral conversion.
- Develop and maintain excellent relationships with stakeholders, including prospective patients, family members, physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives.
- Provide feedback and recommendations for program development and quality improvement initiatives related to customer service, the referral and admission process, and patient and referral source satisfaction.
- Education: Degree from an accredited college or university in a health field, preferably nursing, with current licensure in nursing or respiratory therapy.
- Experience: Minimum of two years' experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.
