Job Summary

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, providing assistance to community nurses, caregivers, and medication personnel to ensure regulatory compliance and clinical excellence.

Key Responsibilities
  • Assist in coordination of the resident admission process, including screening, assessment, and care needs
  • Ensure care is delivered as indicated in resident care plans
  • Medication and treatment administration, ensuring medications are administered per physicians' orders
  • Assistance with coordinating care and services with other outside providers, including resident physicians
  • Document and report care provided and resident observations, including unusual or significant changes
  • Maintain a working knowledge of state regulations, policies, and procedures
Qualifications
  • Working knowledge of state regulations, policies, and procedures, dictated for compliance
  • Graduate from an accredited school of practical nursing
  • Current nursing license and state registration (LPN or RN)
  • Three years' experience in assessment and geriatric nursing, preferred
  • A big smile, eagerness to learn, and compassion for our Residents
