We are seeking a highly skilled and compassionate Nurse Navigator to join our team at HCA Florida Palm Beach Surgical Specialists. As a key member of our multidisciplinary team, you will play a vital role in ensuring the delivery of high-quality patient care and exceptional patient experiences.
Key Responsibilities- Function as a patient advocate from diagnosis to end of active treatment, providing education, support, and resources to patients and families.
- Assess patients' medical, social, and psychosocial needs/barriers to care and link them to appropriate resources.
- Provide teaching, outreach, and support to patients and families, empowering them to manage their own healthcare needs.
- Conduct follow-up conversations with patients and communicate any concerns, changes, or social needs to the appropriate physician or other care providers.
- Three to five years of oncology nursing experience.
- Experience in program development preferred.
- Experience as a nurse navigator and/or patient educator in oncology preferred.
- Experience in leadership and customer service preferred.
- Associate's Degree in Nursing required.
- Bachelor's Degree in Nursing preferred.
- Basic Cardiac Life Support (BCLS) certification.
- Current licensure as a Registered Nurse (RN) in the state of FL.
- Oncology Certified Nurse (OCN) achieved or in progress preferred.
