We are looking for a highly motivated and compassionate Registered Nurse (RN) to provide high-quality home health care services to our patients. The ideal candidate will have experience in home health care and be proficient in using Kinnser/Wellsky EMR systems.

Responsibilities
  • Perform OASIS assessments and develop care plans to address complex chronic illnesses, disabilities, or developmental disabilities.
  • Coordinate the provision of home nursing care, therapy, supportive services, and ancillary services and resources based on our protocol.
  • Provide nursing care through the competent application of the nursing process.
  • Ensure patients have access to services and opportunities that enhance their development, autonomy, and independence, productivity, well-being, and capacity for social interaction.
Requirements
  • 1 year of experience in home health care.
  • 1 year of experience with EMR systems.
  • RN License.
