Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will provide high-quality patient care in post-acute facilities, working closely with our team to ensure the best possible outcomes.

Responsibilities
  • Provide compassionate nursing care to patients in post-acute facilities
  • Monitor patients' physical and mental conditions, tracking vital signs and documenting medical histories
  • Administer medications, track their use and effects, and maintain accurate medical records
  • Cultivate a safe and comfortable environment for patients, working with staff to ensure a smooth and efficient care process
  • Assist with the setup of IVs, immunizations, and phlebotomy procedures, and maintain a proper inventory of medical supplies
  • Work with staff to orient and train new colleagues, ensuring a seamless transition and high-quality patient care
Requirements
  • Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience as an LPN or in a similar role
  • Ability to lift a minimum of 50 lbs
