Nurse - Peritoneal Dialysis Specialist

We are seeking a skilled and compassionate Registered Nurse to join our team as a Peritoneal Dialysis Registered Nurse. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and education to patients with end-stage renal disease.
Key Responsibilities:
- Provide comprehensive patient care in an independent setting, including assessing, diagnosing, and implementing complete care plans.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Provide education and training to patients and their families on peritoneal dialysis, including proper technique and management of the dialysis process.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
Requirements:
- Current Registered Nurse (RN) license in the state of practice.
- Minimum of 12 months of RN experience.
- Current driver's license, reliable transportation, and vehicle insurance in state of residence.
- Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree.
- Basic computer skills and proficiency in MS Word and Outlook.
Preferred Qualifications:
- Previous experience in nephrology nursing or dialysis care.
- Training/Teaching/Healthcare Education background.
- Previous case management experience.
- Familiarity with telehealth technologies.
