We are seeking a skilled Physical Therapist Assistant to join our team at Interim HealthCare of the Twin Cities. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care in a home environment.
Key Responsibilities- Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Provide services that are ordered by the physician as indicated in the plan of care.
- Assist the treating physical therapist in documenting a patient's progress and response to treatment.
- Complete and submit timely, legible, accurate, relevant and sufficient documentation addressing the treatment and interventions provided, the patient's response to the treatment and interventions, and the patient's progress toward goals in accordance with professional standards of practice, policy and procedures, and payer requirements.
- Counsel and educate the patient and the family, and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare related needs.
- Currently licensed as a Physical Therapist Assistant in the State of Minnesota. License is active and in good standing.
- Two (2) years of experience as a PTA preferred.
- Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance.
- CPR Certification.
- Hourly Range: $32.00
- Mileage Reimbursement / Company Car Program
