About the Role

Key Responsibilities
  • Develop and implement patient care plans in collaboration with the healthcare team
  • Provide emotional support and counseling to patients and their families
  • Conduct thorough assessments of patients' social and emotional needs

  • Document patient progress and maintain accurate records
Requirements
  • Master's degree in Social Work from a graduate school accredited by the Council of Social Work
  • 1-2 years of post-graduate experience in a healthcare setting
  • Current license as a Medical Social Worker issued by the state in which you work
  • Valid driver's license and ability to spend approximately 20% of your day driving to/from patient locations
  • Strong time management and organizational skills
  • Experience with EMR software and mobile devices
