Job Title: Case Manager NRI

Community Care Alliance is seeking a skilled Case Manager to join our team. As a Case Manager, you will provide case management services to individuals residing at the North Smithfield Shelter, assisting them in stabilizing their situation and transitioning to independent living.

Key Responsibilities:
  • Deliver case management services to assigned clients, including referrals and care coordination.
  • Complete all required documentation promptly and maintain accurate records.
  • Carry a caseload of 12-15 individuals as needed, providing individualized support and guidance.
  • Assist in room inspections, floor meetings, and conflict resolution, promoting a safe and respectful environment.
  • Attend team meetings, agency-wide meetings, and community meetings as assigned, staying informed and engaged.
  • Conduct case conferences related to client progress, ensuring effective communication and collaboration.
  • Assist in Performance Quality Improvement efforts related to shelter programs, driving continuous improvement.
Requirements:
  • High School Diploma/GED and relevant experience, with a strong interest in ongoing training and professional development.
  • Ability to work independently and within a team, with excellent communication and interpersonal skills.
  • Bi-lingual skills are welcome, but not required.
