Job Title: Nursing Care Specialist - Home Health

About the Role:

We are seeking a skilled and compassionate Nursing Care Specialist to join our team at Ambercare. As a key member of our home health care team, you will provide high-quality nursing care to patients in their homes, promoting independence, dignity, and overall well-being.

Key Responsibilities:

  • Deliver professional nursing care to patients in accordance with established plans of care and company policies and procedures.
  • Assess, plan, and intervene using the nursing process to meet patients' healthcare needs.
  • Observe and report any signs or symptoms indicative of changes in patient condition, documenting reports and observations in clinical records and notifying supervisors as necessary.
  • Perform assigned nursing procedures, including administration of treatments and medications, in accordance with state nurse practice acts and plans of care.
  • Teach patients/caregivers appropriate self-care techniques and educate them on disease management and prevention.
  • Notify case managers, physicians, and families of changes in patient condition and participate in multidisciplinary patient case conferences, staff meetings, and in-service education programs as assigned.
  • Maintain accurate and timely documentation of patient data, adhering to relevant laws, regulations, and agency standards.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of services.
  • Adhere to all relevant laws, regulations, and agency standards, maintaining confidentiality and upholding the highest standards of professionalism.

Requirements and Qualifications:

  • Graduate from an accredited registered nursing program
  • Licensed as a RN in the state of practice
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation, or medical surgical nursing
  • Home health experience preferred
  • Ability to make home visits and provide nursing care in the home environment
  • Ability to complete necessary documentation accurately, legibly, and timely
  • Excellent interpersonal, verbal, and written communication skills

Education and Training:

  • Current licensure as a Registered Nurse in the state of practice
  • Ongoing education and training in areas such as medication management, wound care, and disease-specific interventions

Salary Information:

The estimated annual salary for this position is $65,000-$85,000, depending on location and qualifications. This range reflects industry standards and takes into account factors such as experience, education, and certification.
