Responsibilities:- Evaluate patients before admission and provide medical oversight to ensure seamless care transitions.
- Develop and implement individualized plans of care, including medication management and disease management strategies.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care coordination.
- Maintain accurate and up-to-date patient records, including medication reconciliation and education.
- Participate in process improvement initiatives to optimize care delivery and patient outcomes.
- Nurse Practitioner License for Kentucky and/or Ohio
- Ability to work independently and collaboratively in a fast-paced environment
- Excellent communication and interpersonal skills
- Proficiency in teach-back methods and medication reconciliation
- Flexibility to adapt to changing patient needs and priorities
Monday to Friday, approximately 9 hours per day, with flexible hours and no on-call requirements.
