Job Summary:

Aveanna Healthcare is seeking a skilled Licensed Practical Nurse (LPN/LVN) to join our team. As a LPN/LVN, you will provide high-quality patient care in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices and industry developments.

Requirements:

  • Current, unrestricted LPN/LVN license in the state of practice.
  • Current CPR certification.
  • One year of experience as a LPN/LVN, preferably in home health care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
