Key Responsibilities
  • Social Support Navigation: Collaborate with primary care teams to identify and address social determinants of health, such as food insecurity and housing insecurity.
  • Resource Development: Compile and maintain a comprehensive list of resources for social determinants of health, including eligibility criteria, referral processes, and contact information.
  • Case Management: Conduct case management activities, including discharge planning, follow-up, and education, to ensure seamless patient care.
  • Specialty Referral Navigation: Assist patients in navigating specialty referrals, including scheduling, coordination, and tracking.
  • Patient Education: Provide patient education and connect patients with relevant resources to support their health and well-being.
Expectations
  • Onboarding and Orientation: Complete all necessary onboarding and orientation processes.
  • Team Collaboration: Participate in regional office and primary care clinical meetings as requested.
  • Community Engagement: Attend Health Councils and other community meetings to build relationships with social service agencies and promote health department services.
