Responsibilities:

  • Provide skilled nursing care to patients in their homes, adhering to individualized care plans.">
  • Conduct thorough assessments of patients' physical and emotional needs.">
  • Develop and implement effective care plans in collaboration with patients, families, and healthcare teams.">
  • Monitor patients' progress, adjust care plans as needed, and communicate effectively with healthcare professionals.">
  • Maintain accurate and detailed records of patient interactions, treatments, and outcomes.">

Requirements:

To be successful in this role, you must have:

  • A current RN or LPN license in the state of Wisconsin.">
  • At least 1 year of experience working in pediatrics or a related field.">
  • Strong communication and interpersonal skills.">
  • The ability to work independently and as part of a team.">
  • Flexibility to work varied shifts, including nights.">
