Key Responsibilities- Case Management: Provide comprehensive case management services to patients, including assessment, planning, implementation, and evaluation of care plans to ensure optimal health outcomes.
- Utilization Review: Conduct thorough reviews of patient records to ensure that hospital stays are medically necessary and that patients are receiving the most appropriate level of care.
- Communication: Collaborate with healthcare providers, patients, and families to ensure seamless communication and coordination of care.
- Problem-Solving: Identify and address potential issues or barriers to care, and develop creative solutions to ensure that patients receive the care they need.
- Quality Improvement: Participate in quality improvement initiatives to identify areas for improvement and develop strategies to enhance the quality of care.
- Education: Bachelor's degree in Nursing or related field.
- Experience: 2+ years of experience in case management or utilization review, preferably in an acute care hospital setting.
- Certifications: Current RN licensure and certification in case management or utilization review (e.g. CCM, ACM).
- Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders. Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
