We are seeking a highly skilled and compassionate Registered Nurse to join our team at HomeHealth Advantage Inc. as a Home Health Registered Nurse. As a key member of our healthcare team, you will provide high-quality patient care and education to patients and their caregivers in the comfort of their own homes.
Responsibilities:- Develop and implement individualized care plans for patients with complex medical needs.
- Conduct thorough assessments of patients' physical and emotional needs, and provide education and support to patients and their caregivers.
- Collaborate with healthcare providers and other disciplines to ensure seamless care coordination and continuity.
- Provide hands-on care and management as needed, and evaluate patient progress and adjust care plans accordingly.
- Coordinate with other healthcare professionals to ensure quality services are provided and there is continuity of care among providers.
- Associate's degree in Nursing or higher.
- Valid RN license in the state of employment.
- Valid driver's license.
- Able and willing to travel.
- Ability to successfully complete required background check and drug screen.
- Excellent writing and charting skills, experience with electronic medical records preferred.
