Key Responsibilities:- Practice as a provider member within the Home and Community interdisciplinary team, delivering home-based medical care to patients in assigned geographies.
- Collaborate with primary care physicians, facility staff, and other healthcare professionals to develop and implement patient care plans.
- Conduct comprehensive assessments, develop care plans, and provide education to patients and their families.
- Prescribe diagnostics and interventions to avoid unnecessary transfers or acute admissions.
- Participate in quality improvement initiatives and contribute to the development of new practice ideas and initiatives.
- Completion of a Nurse Practitioner or Physician Assistant accredited program.
- Current state RN license (if Nurse Practitioner) or NP or PA license.
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
- Knowledge of long-term care, community resources, and cost-effective alternatives.
