Key Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
  • Conduct thorough assessments of patients' medical needs and develop personalized care plans to ensure optimal outcomes.
  • Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to coordinate patient care and services.
  • Document patient progress and outcomes in a timely and accurate manner, ensuring compliance with regulatory requirements.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current RN licensure in the applicable state.
  • One year of experience in a patient care setting, with a preference for home health or geriatric experience.
  • Valid driver's license and acceptable driving record.
  • Current CPR certification.
