Job Summary

PruittHealth is seeking a highly skilled Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide holistic, compassionate care to patients and families, and coordinate care management activities on behalf of the PruittHealth Premier Plan.

Key Responsibilities
  • Conduct comprehensive, adult/geriatric physical exams and diagnose chronic diseases in adults.
  • Recognize conditions, characteristics, and behaviors that place seniors at risk and develop effective care plans.
  • Pursue ongoing education and professional development to stay current with best practices.
  • Effectively reconcile medications and identify the needs of clinically complex patients.
  • Work collaboratively with physicians, case managers, social workers, family members, and other healthcare professionals to ensure seamless care coordination.
  • Participate in Quality Assurance Performance Improvement (QAPI) activities and understand the PruittHealth Premier Plan benefits, policies, and procedures.
  • Assess the medical necessity and effectiveness of ancillary services and communicate the process to providers and team members.
  • Coordinate care as members transition through different levels of care and care settings, ensuring that the Individualized Care Plan (ICP) is communicated immediately and/or no later than 24 hours to the receiving care setting.
Requirements
  • Graduate of an accredited Adult or Family Nurse Practitioner Master's Degree program.
  • Minimum 2 years clinical experience as a Nurse Practitioner in geriatric, adult, or family practice setting.
  • Current and unrestricted RN and NP licensure in the state of practice.
  • Board certification by AANP or ANCC, in Adult, Family Practice, and Geriatrics or Adult-Gerontology Primary Care.
Preferred Qualifications
  • Graduate of an accredited Geriatric, Adult, or Family Nurse Practitioner Master's Degree program.
  • Previous long-term care experience strongly preferred.
Knowledge, Skills, and Abilities
  • Pertinent clinical work experience relevant to the population being served.
  • Reviews and interprets medical and social history of members.
  • Interprets laboratory data, recommends appropriate follow-up, and reconciles medications.
  • Manages both medical and behavioral chronic and acute conditions effectively in collaboration with a physician or specialty provider.
  • Ensures that individual plan of care is developed with specific interventions designed to meet the needs identified in the Health Risk Assessment.
