Job Summary

Advocate Aurora Health is seeking a skilled Physical Therapist to join our team in Hinsdale, Illinois. As a Physical Therapist, you will be responsible for providing high-quality patient care and developing individualized treatment plans to achieve optimal outcomes.

Key Responsibilities
  • Interpret physician referrals and complete comprehensive evaluations to identify patient needs and develop effective treatment plans.
  • Implement individualized treatment plans, monitor patient progress, and adjust plans as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Maintain accurate and timely documentation, including patient evaluations, progress notes, and discharge summaries.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care and achieve quality metrics.
Requirements
  • Doctorate degree in Physical Therapy from an accredited institution.
  • Current Physical Therapy License in the State of Illinois or Wisconsin.
  • Basic Life Support (BLS) certification.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
