Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our team at McLeod Health. As a Nurse Care Coordinator, you will play a critical role in ensuring the delivery of high-quality patient care.

Key Responsibilities
  • Assess and implement patient care plans using remote care technology to achieve optimal clinical outcomes.
  • Direct and supervise Licensed Practical Nurses to ensure quality patient care.
  • Collaborate with interdisciplinary teams, including RNs, LPNs, HHAs, PT, OT, ST, and MSW, to coordinate patient care.
  • Provide field supervision of HHA staff and work interdependently with other departments, including Referral-Intake-Scheduling-Authorization (RISA) staff, managers, Practice Support, Billing Specialists, and clerical support.
  • Stay up-to-date with State Licensure Laws, Conditions of Participation for Home Health Agencies, and other relevant regulations.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care coordination.
  • Utilize strong communication and interpersonal skills to effectively manage patient care and resolve any issues that may arise.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • At least two years of nursing experience, with a preference for experience in home care.
  • Current valid SC driver's license and automobile insurance.
  • RN License and BLS Certification.
  • Professional liability insurance is encouraged.
Work Environment

This role will be based in Chesterfield County, working closely with other staff, physicians, and external agencies to provide comprehensive patient care.
