We are seeking a skilled and compassionate Nurse Manager to join our team at the Karen Ann Quinlan Memorial Foundation. As a key member of our hospice care team, you will be responsible for ensuring the delivery of high-quality patient care and services.
Key Responsibilities:- Develop and implement clinical policies and procedures to ensure resident rights and quality care.
- Participate in the hospice quality improvement program, identifying opportunities for improvement and implementing recommendations.
- Provide hands-on patient care, education, and training to staff, ensuring compliance with regulations and standards.
- Lead and develop direct reports, providing coaching, training, and corrective action as needed.
- Maintain liaison relationships with facility staff, services, and residents, representing the Home for Hospice in outside venues as requested.
- Coordinate services to ensure continuity of safe and effective patient care, participating in interdisciplinary team conferences.
- Consult with physicians, hospice team members, patients, and families to establish coordinated hospice referrals.
- Communicate effectively with patients, families, staff, and physicians to ensure delivery of quality care and services.
- Ensure compliance with Medicare, NJ Department of Health, and CHAP regulations.
- Facilitate cohesiveness and open communication within the organization.
- Serve as a customer service advocate, facilitating rapid response to referrals and patient needs.
- Communicate with Intake Coordinator to prioritize admissions and ensure patients meet hospice service criteria.
- Review and ensure accuracy of new admission chart generation and computer data.
- Review documentation in patient charts to ensure compliance with Conditions of Participation, CHAP, and NJDHSS standards.
- Knowledge of hospice conditions of participation, inpatient hospice regulations, and related policies and procedures.
- Clinical knowledge to support the dying process.
- Strong leadership ability.
- Excellent interpersonal and communication skills.
- Ability to comprehend and communicate in the English language.
- Well-organized to manage patient and family needs, provide support to the clinical team, and oversee inpatient hospice operations.
- Self-starter with the ability to work independently and as part of an interdisciplinary team.
- Strong assessment and documentation skills.
- Proficiency in operating a computer, including Microsoft applications and an electronic medical record.
Required Education: Associate degree in Nursing, Licensed to practice as a Registered Nurse in the State of New Jersey, CPR/AED Certification.
Preferred Education: Bachelor of Science in Nursing, CHPN Certification.
Required Experience: Minimum one year of clinical experience in an acute-care setting.
Preferred Experience: Prior hospice and hospice supervisory experience.
