Job Summary

The Southcentral Foundation is seeking a skilled Community Case Manager to join our Behavioral Health Adult Outpatient Services team. As a key member of our multidisciplinary team, you will provide support services to our customer-owners, assisting them with community resources and developing and executing personalized service plans to enhance their daily lives and promote healthy functioning.

Responsibilities
  • Provide direct customer-owner care in a behavioral health setting, utilizing a trauma-informed, systems-based, and person-centered approach.
  • Develop and execute customer-owner service plans, ensuring a comprehensive and coordinated approach to meet their unique needs.
  • Collaborate with multidisciplinary teams, including clinicians and other healthcare professionals, to provide holistic care and support.
  • Assist customer-owners in accessing community resources, including individual psychotherapy, group therapy, and couples therapy.
  • Meet the qualifications and requirements for an approved SCF driver, including a valid Alaska driver's license and a clean driving record.
Requirements
  • BA/BS degree in a social services field of study and one year of work experience providing direct customer-owner care in a behavioral health setting, or an equivalent combination of education and experience.
  • One additional year of professional and supervised social service or related work experience, or an equivalent combination of education and experience, or demonstrated proficiency as a Behavioral Health Case Manager/Behavioral Health Technician II at SCF.
  • Must meet the requirements for an approved SCF driver, including a valid Alaska driver's license and a clean driving record.
Native Preference

The Southcentral Foundation pursues a policy of Native preference in hiring, contracting, and training, as required by P.L. 93-638, as amended. SCF Human Resources must receive certification before applicants receive preference.
