As a key member of our interdisciplinary treatment team, you will play a vital role in facilitating and participating in a multi-disciplinary treatment team, providing weekly supervision to program employees, and conducting biopsychosocial assessments and ongoing clinical evaluations.
Key Responsibilities- Facilitate and Participate in Treatment Team Meetings: Collaborate with a multi-disciplinary team to develop and implement treatment plans for program participants.
- Provide Supervision and Support: Offer weekly supervision and guidance to program employees to ensure they are equipped to provide high-quality care.
- Conduct Clinical Assessments: Conduct biopsychosocial assessments and ongoing clinical evaluations to inform treatment planning and ensure program participants receive the best possible care.
- Communicate and Collaborate with Stakeholders: Regularly communicate and collaborate with the IOC court system, IOC team, physicians, court staff, judges, public defenders, and other legal teams to reduce the risk of rehospitalization and ensure program participants comply with treatment recommendations.
- Engage Consumers in Treatment Planning: Work closely with program participants to develop and implement individualized treatment plans that address their unique needs and goals.
- Provide Direct Care and Support: As a member of the treatment team, provide direct care and support to program participants in their homes and communities.
- Assist with Daily Living Activities: Assist program participants with activities of daily living, such as modeling, direct assistance with ADLs, and coaching.
- Provide Crisis Intervention: Respond to crisis situations as needed and provide support to program participants in times of need.
- On-Call Coverage: Provide 24-hour on-call coverage to ensure program participants receive support and guidance at all times.
- Master's Degree in Social Work, Psychology, or Related Field: Possess a master's degree in social work, psychology, or a related field.
- LSW/LAC Required: Hold a Licensed Social Worker (LSW) or Licensed Associate Counselor (LAC) credential.
- LCSW/LPC Preferred: Possess a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) credential.
- Experience Working with SMI/MICA Population: Have experience working with individuals with serious mental illness (SMI) and/or mental illness and co-occurring substance use disorder (MICA).
- Excellent Communication and Interpersonal Skills: Possess excellent written, verbal, and interpersonal communication skills.
- Valid NJ Driver's License: Hold a valid New Jersey driver's license with fewer than six points.
