Job Summary
The RN Case Manager is responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence. This role functions independently and/or in conjunction with an interdisciplinary team based on the patient's identified needs.
Key Responsibilities
- Provide comprehensive nursing services to patients in their homes, including wound care, medication management, and education.
- Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress.
- Document patient information and care plans in a timely and accurate manner.
Requirements
- Graduate of an accredited school of nursing.
- Current Washington State Registered Nurse License.
- National Provider BLS - American Heart Association certification.
- National Provider Identifier (NPI) number.
- Preferred qualifications include a Bachelor's Degree, 2 years of full-time experience practicing as a registered nurse, and recent hands-on technical clinical experience in a Medicare-certified Homecare or Hospice agency/facility.
