Description:

Sky View Rehabilitation and Health Care, a 192-bed Skilled Nursing Facility, has an opening for a PRN Physical Therapist. Our luxurious and hotel-like environment is the ideal setting for those in need of Post Acute Rehabilitation or Long-Term Care.

Our team of highly skilled physical, occupational, speech, and respiratory therapists work together to create an individualized therapy plan that helps residents reach their highest level of functioning in as short an amount of time as possible.

Responsibilities:
  • Assist in planning, organizing, developing, and implementing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations.
  • Plan, develop, organize, evaluate, and implement the programs, goals, objectives, policies, and procedures of the physical therapy program to help residents obtain/maintain their highest level of physical, mental, and social well-being.
  • Interpret and/or instruct in physical therapy policies and procedures to personnel, residents, family members, etc. as necessary.
  • Participate in discharge planning, development, and implementation of resident care plans, resident assessments (MDS), etc. as necessary.
  • Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
  • Maintain treatment records, resident files, and progress notes as required and maintain an adequate liaison with families and residents.
Requirements:
  • Must possess a current license and registration to practice as a Physical Therapist, PT in the State of NY.
  • Must possess current malpractice insurance, license in good standing.
  • Dedicated to providing extraordinary compassionate care as a Physical Therapist, PT.
  • Energetic, enthusiastic, committed, and passionate about quality care.
  • Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents.
  • Patience, a cheerful disposition, and willingness to assist any residents and co-workers.
  • Excellent communication, customer service, and organization skills.
  • Trusted to work independently and in a team environment.
  • Able to work well under pressure, flexible, and will work harmoniously with all staff.
  • Covid-19 vaccinations are encouraged but NOT MANDATED; we continue to offer COVID-19 vaccination based on eligibility guidance from CDC.
