Healthcare Coach

Primecare Home Care Services is seeking a dedicated and compassionate Admin Health Coach to support family caregivers and their care recipients. As a key member of our team, you will provide empathetic support, education, and guidance to caregivers, empowering them to manage their own psychosocial, physical, and environmental health to improve and maintain wellbeing.

• Engage and motivate caregivers through phone and digital technology, empowering them to manage their own health and wellbeing.
• Manage a caseload and leverage technology to enable effective coaching and information referral.
• Process payroll for your caseload on a weekly basis.
• Assess caregiver and care recipient needs, assisting the caregiver in creating and achieving care plan goals.
• Collaborate with key stakeholders to assess care/coaching plans and caregiver engagement, identifying problems and solutions.
• Document and report on coaching activity with a focus on quality and outcomes, making recommendations as appropriate.
• Perform case conferences, summary reports, and incident management in compliance with applicable state and program regulatory and/or accreditation requirements.
• Solicit feedback from caregivers to identify opportunities for additional support and process improvement.

• 2 years experience in case management, experience working with elderly and disabled.
• Exceptional active listening and communication skills with strong empathy.
• Tech savvy with comfort using (and teaching others to use) mobile apps.
• Ability to multitask in a multi-system environment; proficiency with Microsoft Office required.
• Thrives in a fast-paced, start-up environment.
• Solid understanding and experience in care planning/case management is required.
• Multilingual skills are highly desired.
