We are seeking a skilled Pediatric Case Manager to join our team at Pyramid Consulting, Inc. This is a contract-to-hire opportunity with long-term potential and is located in Tampa, FL.

Key Responsibilities:
  • Provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and providing community resources to address social determinants of health needs.
  • Manage a caseload of 100 members and document in an electronic health record.
  • Comply with care management policies/procedures, including outreach and documentation expectations.
  • Participate in performance expectations/metrics for this individual and their team.
Requirements and Technology Experience:
  • Pediatric case management experience.
  • Experience connecting families/children to local resources for various services, including employment, housing, independent living, justice, foster care, etc.
  • Health plan experience.
  • Foster care and/or adoption experience.
  • Bachelor's Degree.
  • Microsoft Office- Outlook, Word, and Excel.
