At Choice Home Health, we are seeking a skilled and compassionate Registered Nurse to join our team. As a Registered Nurse, you will play a vital role in providing high-quality patient care and promoting independence in the comfort of patients' homes.
Job Summary:We are looking for a dedicated and experienced Registered Nurse to develop and implement individualized care plans for patients with chronic conditions, illnesses, or injuries. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
Responsibilities:- Conduct comprehensive assessments of patients' needs and develop personalized care plans in collaboration with patients, caregivers, and healthcare providers.
- Provide skilled nursing care, including wound care, medication management, and education to patients and caregivers.
- Monitor patients' conditions, assess their wounds, and provide regular updates to healthcare providers.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Document patient information accurately and timely, including progress notes, medication profiles, and re-certification documentation.
- Graduation from an accredited nursing program and current state license as a Registered Nurse.
- At least one year of clinical practical nursing experience, with home health experience preferred.
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
- Ability to make home visits and provide skilled nursing assessments and care in the home.
