Nursing Professional for Long-Term Care

Amergis is seeking a skilled Nursing Professional to join our team in a Long-Term Care setting.
**Salary:** $33 per hour, paid weekly
Job Description:
We are looking for a dedicated and compassionate nurse to provide high-quality patient care in a Long-Term Care/Assisted Living Facility. As a key member of our team, you will be responsible for assessing, planning, implementing, and evaluating patient care.
Key Responsibilities:
- Utilize the nursing process to assess, plan, implement, and evaluate patient care
- Perform focused interviews to identify specific patient needs
- Assess signs and symptoms indicating physiologic and psychological changes in patients' conditions
- Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses
- Document patients' plans of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions
- Identify and prioritize actions based on patient care requirements
- Monitor consistency between patient plans of care and actions of medical staff
- Perform interventions according to identified priorities, plans of care, and facility policies and patient outcome standards
- Provide a safe, comfortable, and therapeutic patient environment
- Initiate and evaluate patient and/or family education
- Revises plans of care according to evaluation, changes in medical plans of care, and effective/ineffective nursing interventions
- Document patients' responses to nursing and other interventions
- Use clinical judgment in evaluation activities to meet patient care needs of assigned floors/units, including establishing priorities
- Monitor safety and effectiveness of the environment and equipment
- Participate in activities to facilitate good interpersonal communication with chosen facilities
- Support professional practice by observing the following: uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patients' Bill of Rights; and 6) Amergis Healthcare, nursing, and client hospital policies
- Perform other duties as assigned/necessary
Requirements:
- Current Licensed Practical Nurse License for the state in which the nurse practices
- Ability to effectively elicit/provide information to and from appropriate individuals via strong spoken, written, listening, and nonverbal communication skills
- Current Health Certificate (if applicable)
- Current PPD or Chest X-Ray
- One year prior professional nursing experience preferred
- Current BLS card
- Successful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greater
- Successful completion of Background Screening Process
