We are seeking a skilled Physical Therapist to join our team at Hackensack Meridian Health. As a Physical Therapist, you will play a vital role in the provision of physical therapy services to our patients.
Key Responsibilities- Support the Mission, Values, and Philosophy of the HMH network.
- Adhere to HMH Organizational competencies and Standards of Behavior.
- Evaluate and treat an assigned caseload of patients, utilizing evidence-based practices to promote optimal patient outcomes.
- Maintain accurate and detailed documentation of patient care, adhering to regulatory and departmental standards.
- Participate in the Interdisciplinary Team process, collaborating with healthcare professionals to develop and implement comprehensive patient care plans.
- Assist in carrying out the performance improvement plans of the facility/department, contributing to the continuous quality improvement of our services.
- Participate in continuing education programs, staying current with best practices and advancements in the field of physical therapy.
- Support the goals and objectives of the department, contributing to the achievement of our strategic objectives.
- Participate in the process of Performance Management, receiving and providing feedback to ensure individual and team performance excellence.
- Provide supervision and direction to Physical Therapist Assistants, physical therapy students, therapy technicians, and volunteers, promoting a culture of teamwork and collaboration.
- Perform other duties and/or projects as assigned, contributing to the overall success of the department and the organization.
- Graduate of an accredited physical therapy program or equivalent.
- Minimum of two years of clinical experience, with a strong background in physical therapy services.
- Advance certifications in physical therapy, such as board certification or specialized training.
- Maintain a current New Jersey PT License, adhering to all regulatory requirements.
