About the Role
We're seeking an experienced Primary Care Physician to join our team of healthcare professionals. Your primary responsibility will be to provide comprehensive healthcare services to our patients, focusing on accountability for patient outcomes and acting as a health coach. You'll lead a care team, engage with patients experiencing acute and chronic illnesses, and work collaboratively to meet quality care metrics.
Key Responsibilities
- Independently provide care for patients with acute and chronic illnesses
- Take full accountability for patient care and outcomes
- Engage with hospitalists whenever one of your patients is in the hospital
- Lead your care team consisting of care promoters, care facilitators, and care coordinators
- Perform other duties as assigned by manager's discretion
Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred
- Current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from American Heart Association (AMA) or American Red Cross within first 90 days of employment
