Key Responsibilities- Initial Assessment and Care Planning: Conduct thorough assessments of patients and families to determine hospice needs, and develop individualized care plans that address their unique needs and goals.

- Education and Licensure: Graduate of an accredited nursing school and licensed as a Registered Nurse in the Commonwealth of Virginia.
- Experience and Skills: Minimum 1 year of experience in hospice care or a related field, with excellent communication and interpersonal skills.
- Preferred Qualifications: Certified Hospice and Palliative Nurse (CHPN) certification, and basic knowledge of Medicare/Medicaid reimbursement regulations.
