Job Overview:
The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) collaborates with physicians, Care Coordinators, registered nurses, and other healthcare professionals to deliver comprehensive and compassionate care in post-acute settings.
This role involves working closely with a physician(s) to diagnose, manage, and prevent both acute and chronic conditions while promoting overall wellness.

Key Responsibilities:
The essential duties include, but are not limited to:
- Conducting thorough health risk assessments for all assigned patients.
- Collaborating with physicians, Care Coordinators, facility staff, family members, and other relevant medical personnel.
- Identifying quality improvement opportunities through tailored healthcare planning.
- Reviewing and interpreting patients' medical, surgical, and social histories.
- Performing necessary screening procedures and interpreting diagnostic data.
- Prescribing and administering various treatment modalities, including medications and wellness counseling.
- Participating in mandatory compliance training and adhering to all relevant regulations and policies.
- Referring patients to primary care providers and management programs as needed.
- Providing constructive feedback to enhance operational efficiency.
- Maintaining a focus on delivering timely customer service.
- Engaging with plan members to build positive relationships.
- Assisting with special projects and participating in client meetings as required.
Minimum Qualifications:
To be considered for this position, candidates must:
- Be fully vaccinated within 90 days of employment.
- Demonstrate proficiency in computer skills, including email and Microsoft Office applications.
- Exhibit strong written and verbal communication skills.
- Have at least one year of clinical experience as a Nurse Practitioner or Physician Assistant.
- Possess the ability to perform comprehensive physical examinations and document chronic diseases in adults.
- Show commitment to ongoing professional development and education.
Physical Requirements:
The physical demands of this role include:
- Regularly talking and hearing.
- Frequently using hands to handle objects.
- Occasionally standing, walking, and sitting.
- Ability to lift and/or move up to 25 pounds.
