Job Summary

At EmblemHealth, we are seeking a skilled Registered Nurse to join our team. As a Registered Nurse, you will be responsible for providing high-quality patient care, including assessment, treatment, and care planning. You will work closely with our healthcare team to ensure that our patients receive the best possible care.

Responsibilities:

  • Provide nursing care to patients, including wound care and medication administration.
  • Verify patient's chief complaint.
  • Take vital signs as requested.
  • Review patient chart for missing lab work, reports, and open orders.
  • Review EHR active medication list with patient. Conduct and document point-of-care tests based on protocol.
  • Administer medications as directed by provider.
  • Provide care in accordance with nursing standards and organizational policy following established procedures for infection control, patient care, and safety in compliance with all federal, state, and local standards (OSHA, HIPAA, etc.).
  • Add recommendations to individualized care plan(s) and review After Visit Summary (AVS) with patient.
  • Document progress notes in electronic health record within 24 hours.
  • Coordinate outpatient work-up and or treatment plans when required for determination of diagnosis (i.e., further clinical testing, x-rays, and facilitate communication between physicians and healthcare providers).
  • Participate in the development of strategies to maintain high levels of wellness for at-risk populations.
  • Teach and support patients with self-care management.
  • Facilitate chronic condition management education to patients.
  • Actively seek input to support patient care and outcomes.
  • Review work to be completed and develop a plan based upon patient/population need, ACP policy/procedure, and applicable rules and regulations.
  • Support implementation of programs and procedures developed at an organizational level.
  • Collect data and adjust process to accommodate program requirements.
  • Participate in ongoing education activities.
  • Manage in-basket requests and respond to all requests in a timely manner.
  • Handle phone calls from patients regarding their questions and addressing their concerns.
  • Handle requests for medication refills according to ACP procedure.
  • Contact patients with test results and provide recommendations on cases per physician determination and/or standards of care.
  • Follow up on abnormal laboratory results requiring patient action and possibly re-measurement (increasing medication dose, dietary changes, etc.).
  • Call patients at doctor's request to assure compliance with treatment plan and/or response to treatment prescribed.
  • Understand and support practice of other team members and refer patients appropriately.
  • Support patient transition through different levels of care (e.g., hospital to home through outreach and assessment, assure timeliness of doctor follow-up).
  • Perform other job-related duties as required.

Qualifications:

  • Completion of nursing training from an accredited school of nursing (R)
  • Current unencumbered New York State Registered Nurse license (R)
  • BLS (R)
  • Work experience as a nurse in a hospital or ambulatory setting (P)
  • Medical office practice experience. (P)
  • Strong knowledge of professional nursing theory, regulations, and practices to give and evaluate patient care. (R)
  • Skill in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care. (R)
  • Ability to maintain quality control standards including knowledge of policies and procedures related to infection control, environmental safety, patient confidentiality, and common safety hazards and precautions to establish/maintain a safe work environment. (R)
  • Excellent verbal and non-verbal communication skills to provide patient care and interact with colleagues in a team atmosphere. (R)
  • Knowledge of electronic medical records (EMR). (P)
