Job Description

Job Title: Team RN, Hospice

Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team. As a Team RN, you will play a vital role in providing high-quality patient care and support to individuals and their families during their end-of-life journey.

Responsibilities:

  • Assess, coordinate, and deliver nursing care to patients in accordance with the Hospice philosophy and Sutter Health policies and procedures.
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support.
  • Provide education and support to patients and their families to promote comfort, dignity, and quality of life.
  • Document patient care and progress in accordance with regulatory requirements and Sutter Health policies.

Requirements:

  • Graduate of an accredited school of nursing.
  • Current RN licensure in the state of California.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and automobile insurance.
  • 2 years of recent relevant experience in hospice or palliative care.

Preferred Qualifications:

  • Experience with Electronic Health Records (EHRs).
  • Knowledge of hospice and palliative care principles and practices.
  • Ability to work independently and as part of a team.

Work Environment:

This position may require regular work in a home setting, with occasional travel to the office or other locations as needed.
