Job Title: Community-Based Case Manager

CareStar, Inc. is seeking a motivated and experienced Community-Based Case Manager to join our Clinical Department. As a key member of our team, you will play a vital role in assisting and enabling individuals to remain in their least restrictive environment while maintaining their independence and dignity.

Responsibilities:
  • Assist participants with access to and coordination of services to enable them to receive a range of appropriate services in a planned, coordinated, efficient, and effective manner.
  • Promote and support participants' preferences, values, and the right to self-determination.
  • Approve funds for utilization of available services, while meeting all qualifications and standards as specified by the program.
  • Develop annual cost budgets.
  • Remain current through continuing education and utilize knowledge base of Medicare, Medicaid, insurance, and state-administered waivers, as well as the Americans with Disabilities Act.
  • Function as the team leader in the development and implementation of the consumer's care plan, including the authorization of the amount, scope, and duration of services.
  • Conduct home visits and/or appropriate contacts and monitors services, in compliance with CareStar Case Management and assessment expectations and the rules specified in the assigned program.
  • Identify and report high-risk situations with consumers and independently take appropriate action to mitigate health risks to the individual.
Requirements:
  • Active license as a Licensed Social Worker, Licensed Independent Social Worker, or Registered Nurse.
  • 12 months of experience in home and community-based services within the past 5 years is preferred.
  • Knowledge of Medicare, Medicaid, home health care, and Ohio Home Care waiver programs is desirable.
