As a Behavioral Health Specialist, you will play a crucial role in conducting clinical assessments, developing personalized behavioral care for patients with obesity, and engaging them in our multidisciplinary service model.
Key Responsibilities:
- Conduct comprehensive clinical assessments and develop tailored behavioral care plans for patients with obesity.
- Collaborate with a care team, including a nurse practitioner, dietitian, exercise trainer, and other clinicians, to provide comprehensive care.
- Work with patients to engage them in our service model and support their behavioral health needs.
Requirements:
- Master's degree in Social Work from an accredited program.
- 1-2 years of experience working with adults and children.
