Job Summary:

At Caregivers Home Health Inc, we are seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for evaluating, planning, and administering comprehensive physical therapy modalities to patients in their place of residence. Your goal will be to develop and restore function through rehabilitative physical therapy as prescribed by a physician.


Duties and Responsibilities:
  • Perform accurate and comprehensive assessments of patients' physical therapy needs.
  • Obtain physician orders for certification and recertification of home physical therapy services.
  • Communicate changes in patients' clinical status to other healthcare team members and Agency office staff in a timely manner.
  • Coordinate care planning and delivery with the physician, patient, and family/caregivers, other healthcare team members, and Agency staff to facilitate optimal patient outcomes.
  • Educate patients, their families/caregivers, and other staff as appropriate in physical therapy modalities.
  • Deliver physical therapy modalities appropriate to patients' clinical status and needs, and in accordance with the standard of care for patients' age.
  • Provide, modify, or discontinue physical therapy modalities based on ongoing assessments/reassessments of patients' clinical status and physician orders.
  • Assess patients for the need for durable medical equipment and other adaptive equipment needs.
  • Develop home exercise programs tailored to patients' clinical status, needs, and age.
  • Synthesize data from physical examinations and analyze data to make clinical judgments regarding patient management.
  • Able to assess and reassess patients' pain.
  • Utilize appropriate pain management modalities, including physical therapy modalities.
  • Educate patients and/or caregivers in effective pain management modalities.
  • Provide physical therapy modalities in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.
  • Identify risks of injuries or threats to life and health in patients' environments and implement appropriate interventions to reduce those risks.
  • Address and support cultural practices that do not harm patients or others and do not interfere with the planned course of physical therapy.
  • Perform transfer and discharge planning functions in accordance with Agency policy and applicable laws, regulations, and standards.
  • Participate in the organization's information management program.
  • Comply with Agency policies and adhere to all applicable laws, regulations, and standards.
  • Complete and submit necessary documentation in accordance with Agency policy and applicable regulations.
  • Submit verification of completed visits in accordance with Agency policy.
  • Participate in interdisciplinary team conferences in accordance with Agency policy.
  • Maintain current knowledge of community resources and identify and access appropriate external and internal resources to meet patients' and/or family needs and facilitate optimal patient outcomes while home care services are being provided, and for transfer and discharge processes.

Regulatory Requirements:
  • Current valid state Physical Therapy license.
  • Graduate of an accredited physical therapy curriculum/school.
  • For Medicare certified agencies, Graduate of a physical therapy curriculum approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.
