Job Summary

This position involves working with patients who require complex care, including those with renal disease. The goal is to facilitate timely discharges and ensure optimal patient outcomes. We are seeking an experienced social worker to join our team at Grace Medical Center.

  • Conduct comprehensive assessments of patients' needs and develop personalized care plans.
  • Work collaboratively with healthcare providers to coordinate care and ensure seamless transitions.
Responsibilities
  • Communicate effectively with patients, families, and healthcare teams to promote understanding and cooperation.
  • Participate in quality improvement initiatives to enhance patient care and satisfaction.
Requirements
  • A Master's degree in Social Work or a related field is required.
  • At least 3 years of experience in healthcare, preferably in a similar role.
  • Current licensure as an LCSW-C or LCPC in the State of Maryland.
