RN Case Manager PRN Job Summary

We are seeking a seasoned and ambitious RN Case Manager to join our team at HCA HealthONE Mountain Ridge. As a key member of our healthcare family, you will play a vital role in coordinating services through the continuum of care, balancing quality patient care with cost-effective resource utilization.

Key Responsibilities:
  • Conduct concurrent clinical reviews to determine appropriateness of admission and continued need for care delivery in the acute hospital setting.
  • Support collaboration and communication among the patient/family and entire healthcare team regarding the patient's plan of care and continued needs post-discharge.
  • Perform telephonic clinical reviews with claims insurance personnel to facilitate reimbursement for the delivery of patient care services.
  • Identify quality and/or risk issues and report to the appropriate individual/department.
Requirements:
  • Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license.
  • Current BLS with American Heart Association or American Red Cross.
  • Case Management or CPHQ Certification strongly preferred.
  • Graduate of an Accredited School of Nursing; BSN desirable.
  • Skill in assessing, planning, and managing patient care delivery as acquired through a minimum of 2 years of clinical nursing experience in the acute care setting.
  • Prior utilization/quality/discharge planning/and/or case management experience highly preferred.
