Job Title: Nurse Practitioner - Advanced Practice Provider

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Oak Street Health. As a Nurse Practitioner - Advanced Practice Provider, you will play a critical role in providing high-quality, patient-centered care to our Medicare patients.

Job Summary:

The Nurse Practitioner - Advanced Practice Provider will be responsible for providing primary care services to patients, including conducting office visits, administering wellness visits and health risk assessments, and coordinating care with other providers and specialists. This role requires a strong understanding of population health and a passion for delivering exceptional patient care.

Key Responsibilities:
  • Provide primary care services to patients, including conducting office visits and administering wellness visits and health risk assessments.
  • Coordinate care with other providers and specialists to ensure comprehensive and coordinated care.
  • Develop and implement care plans that address the unique needs and goals of each patient.
  • Collaborate with the care team to ensure seamless transitions of care and optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA license.
  • US Work Authorization.
Preferred Qualifications:
  • Experience in primary care, internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
  • Passion for teamwork and collaboration.
  • Bilingual proficiency in applicable areas.
