Job Summary

We are seeking a skilled and experienced Physical Therapist to join our team at Davis Memorial Hospital. As a Traveling Physical Therapist, you will have the opportunity to work in a variety of settings and locations, providing high-quality patient care and developing your clinical skills.

Responsibilities
  • Provide physical therapy services to patients in their homes, focusing on rehabilitation and recovery from illness or injury.
  • Conduct thorough assessments and evaluations to identify patient needs and develop personalized treatment plans.
  • Implement and progress treatment plans, using a variety of techniques and modalities to promote patient progress and independence.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
  • Document patient progress and outcomes, using standardized tools and measures to track and report on patient outcomes.
  • Collaborate with other healthcare professionals to develop and implement comprehensive care plans, addressing the physical, emotional, and social needs of patients.
Requirements
  • 1 year of experience as a Physical Therapist in a home health setting.
  • Current licensure as a Physical Therapist in the state of West Virginia.
  • Knowledge of OASIS and EMR systems, including Netsmart (McKesson).
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on patient-centered care and outcomes.
Preferred Qualifications
  • Experience working with patients with a variety of diagnoses and conditions, including orthopedic, neurologic, and cardiovascular conditions.
  • Knowledge of adaptive devices and equipment, including gait belts, hospital beds, and wheelchairs.
  • Ability to work in a fast-paced, dynamic environment, with a focus on patient safety and quality care.
