Job Title: Nurse - REACH

About the Role



As a dedicated and compassionate Nurse, you will play a vital role in providing exceptional care to our clients at the REACH facility. Your primary responsibility will be to deliver high-quality nursing services, ensuring the physical and emotional well-being of our residents. You will work collaboratively with our multidisciplinary team to create a supportive and therapeutic environment that promotes recovery and growth.



Key Responsibilities




  • Provide direct patient care: Administer medications, monitor vital signs, and provide wound care and other nursing interventions as needed.
  • Develop and implement individualized care plans: Collaborate with the treatment team to create personalized care plans that meet the unique needs and goals of each client.
  • Conduct assessments and evaluations: Perform thorough assessments and evaluations to identify client needs, develop treatment plans, and monitor progress.
  • Communicate effectively: Maintain open and honest communication with clients, families, and the treatment team to ensure seamless care coordination.
  • Participate in multidisciplinary team meetings: Contribute to case conferences and other team meetings to discuss client progress, address concerns, and plan for future care.


Requirements




  • Registered Nurse (RN) licensure: Current licensure as a Registered Nurse in the State of Oregon.
  • Experience in mental health nursing: Previous experience working in mental health or a related field is highly preferred.
  • Clinical skills and knowledge: Possess strong clinical skills and knowledge of nursing practices, medications, and treatments.
  • Communication and interpersonal skills: Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and the treatment team.
