Eventus WholeHealth is seeking a compassionate Geriatric Healthcare Specialist to join our team in Somerset, KY. In this role, you will be responsible for providing primary care services to older adults in skilled nursing facilities and assisted living facilities.

Duties and Responsibilities:

  • Conduct comprehensive health assessments of geriatric patients
  • Develop individualized treatment plans to address the unique needs of each patient
  • Order and interpret diagnostic tests to inform treatment decisions
  • Prescribe medications and other treatments as needed
  • Provide patient education and counseling to promote healthy aging
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination
  • Maintain accurate and up-to-date medical records
  • Participate in quality improvement initiatives to enhance patient outcomes

Qualifications:

  • Master's degree in nursing or physician assistant studies
  • Current state licensure as a nurse practitioner or physician assistant
  • Certification as a nurse practitioner or physician assistant
  • Experience in primary care, preferably in a skilled nursing facility or assisted living facility
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Ability to maintain confidentiality and adhere to ethical standards

Estimated Annual Salary:** $125,000 - $155,000
