Responsibilities
  • Conduct thorough assessments of patients' physical and functional abilities to identify areas for improvement
  • Develop and implement individualized treatment plans to address patients' needs and goals
  • Provide occupational therapy services to patients in their homes, including exercises, activities, and education to promote independence and functional ability
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination
  • Document patient progress and outcomes in accordance with company policies and procedures
Requirements
  • Master's degree in Occupational Therapy or equivalent with license or certificate within the state providing services
  • At least two years of experience working independently under minimal supervision
  • Must be a licensed driver who can travel to all business locations
