Millennium Home Care, a leading provider of healthcare services, is seeking a dedicated Speech and Language Pathologist to join our team as a PRN Speech and Language Pathologist. Our team is passionate about making a difference in the lives of our patients and their families.

Responsibilities:
  • Conduct comprehensive evaluations of patients' speech, language, and swallowing abilities to determine their communication and feeding needs.
  • Develop personalized treatment plans based on patients' individual goals and functional limitations.
  • Provide individualized therapy services to patients in their homes, targeting speech and language skills, articulation, voice, fluency, and swallowing disorders.
  • Educate patients and their families on techniques and strategies to improve communication, language skills, and swallowing function.
  • Collaborate with other healthcare professionals, such as physicians, occupational therapists, and physical therapists, to ensure a holistic and coordinated approach to patient care.
  • Document patient progress, treatment plans, and outcomes accurately and in a timely manner.
  • Stay up to date with the latest advancements and evidence-based practices in speech and language pathology through continued professional development.
  • Maintain compliance with all relevant regulations, policies, and procedures.
Qualifications:
  • Valid license as a Speech and Language Pathologist (SLP) in the state of Florida.
  • Minimum of 1 year of experience working as a Speech and Language Pathologist.
  • Strong knowledge of speech and language pathology principles, techniques, and best practices.
  • Excellent interpersonal and communication skills to effectively interact with patients, families, and multidisciplinary teams.
  • Ability to work independently and manage time effectively to meet patients' needs.
  • Proficiency in documenting patient progress and treatment plans using electronic medical records (EMR).
  • Valid driver's license and reliable transportation to travel to patients' homes.
